Make a referral

We accept self-referrals from individuals aged 16-25 and those completed by a parent/carer for individuals under the age of 16.  Health, education, social work and other professionals can also put individuals forward for referral.  For those over the age of 25, please see our page on help for over 25s.

 

To refer yourself, your child, or put someone else forward for a referral, please select from one of the options below.  After submitting the appropriate request form you will be emailed instructions on what to do next.  ​
